If you or your family are in need of help with food, or gifts for the young ones this Holiday Season, please scan the QR code below to access the request form. The form has been changed; there are 4 sections. Instructions are on the form, including pickup times. This single form will now take care of Thanksgiving & Christmas dinner, as well as gift requests for your children.
